Is it bad when your pregnant and your HCG level dropped from 200 to 48?

ok im 8 weeks pregnant..i started to bleed on aug 29th 2009 but it was really light so i didn't think much of it...then on sept 1st i went to the emergency room because i started bleeding heavily he gave me a pelvic exam and said i wasn;t dialating but my HCG level was only at 187 and it should have been more so he told me to rest and to come back on sept 4th to do retest but before that i was still bleedin heavily i used pad but it didnt bleed every hour...but when i went back on sept 4th to do the retest my HCG level dropped to 48 he told me i had or probably was miscarriaging and to see a obgyn but i applied for medicaid but my approve didnt come in yet so what should i do if i dont have money to go to the doctors? will they do an ultrasound and bill medicaid when i get it? i dont know what to do
